💡 What does the ending really mean?

At the end of Smile 2, Skye discovers that her recent escapes were hallucinations and finds herself at her concert. The entity, taking on her form, reveals its true monstrous nature as the artist enters a trance. Finally, Skye stabs a microphone into her eye and dies in front of thousands of spectators.

What happens at the end

The protagonist meets with a nurse named Morris, who proposes temporarily stopping her heart in an abandoned freezer to trick the parasitic entity and end the deadly cycle. Skye manages to inject herself with the substance intended to stop her heartbeat, but the entity manifests in the appearance of her former, accident-prone self and proves to her that she has never been in control of the situation.

Immediately after, reality collapses, and it is revealed that much of the recent events, including the meeting with Morris and her escape, were hallucinations within the singer's mind. Skye finds herself on stage during her comeback concert at Herald Square Garden, seeing her mother alive in the audience. The entity adopts Skye's own form and tears open its stomach to release its true, monstrous nature: a gigantic, skinless being equipped with multiple smiling mouths. At that moment, the artist falls into a final trance and, under the horrified gaze of thousands of spectators, stabs a microphone into her eye and dies.

What it means

The film uses this conclusion to take the concept of trauma and guilt to its most nihilistic and bleak extreme. The final trap of multiple hallucinatory layers functions as a reflection of the paranoia and suffocation suffered by a celebrity hounded both by her own past mistakes and by the relentless machinery of fame. The entity abandons its subtle forms to reveal itself as a grotesque parasite that devours the protagonist's public and private identity on live television.

What is left open

The film does not reveal whether there is any real method to stop the cycle of contagion, given that all the characters' desperate attempts to outsmart the parasite inevitably end in a macabre dead end.
